international Qualifications and Required Documents

Applicants from outside of the United States and Canada must provide the following additional documentation in order to be forwarded to the Membership Committee for review:

• Certificate or verification from government agency stating when the business was established and that the business is currently in good standing.
• List of locations including contact information and the number of dart machines (minimum of four locations).
• Serial number of each dart machine in each location.
• Certificate of sale for each dart machine or contact information of the distributor from whom dart machines were purchased.
• List of additional coin amusement music or vending machines in operation in each location.
• Copies of the following documentation must also be provided if currently running leagues or at the time that leagues are implemented:
  • Copies of all league schedules
  • Samples of scoresheets used in league play
  • Copy of league financial report
  • Copies of league statistics for each league session from the first, middle and last week of each league season that includes the following information: player names, number of games played for each player, skill rating using PPD and/or MPR for each player
